According to this Waiver and Release Agreement (hereinafter referred to as “Agreement”), I hereby acknowledge and agree that: (i) in order for me to be allowed to participate in the Sixth NTD International Piano Competition (hereinafter referred to as “Competition”), I give up my rights to hold the organizers, and their employees, agents, officers, directors, managers and judges (the “Covered Entities”) liable for any injury or damage that I may suffer while participating in the Competition; (ii) I will release, discharge, waive and forever relinquish any and all actions, causes of action and claims that I have, may have or have had, whether past, present or future, whether known or unknown, and whether anticipated or unanticipated by me, arising out of participation in the Competition; and (iii) by signing this Agreement, I am assuming full responsibility for any and all risk while participating in the Competition. I am committed to follow the instructions and rules set by the organizers, and I will not withdraw at any time from the competition without the consent of the organizers. I agree to allow the organizers to use my biography and resume, all my photos and video footages during the entire competition to be used for promotional and commercial purposes. I promise to provide correct information about myself. I have the responsibility to explain and correct any ambiguity or inaccuracy concerning my personal data during the competition process. By entering into this Agreement, I am not relying on any oral or written representation or statements made by any person, other than what is set forth in this Agreement. I agree that this Agreement shall be governed by and interpreted in accordance with the laws of the State of New York, U.S.A. I hereby declare that I am the contestant. I have read this agreement, understand it and agree to be bound to it as a contract. (Signature of guardian is required if applicant is under 18 years old.) 本人同意 1）參加新唐人電視台2021第六屆「國際鋼琴大賽」（以下簡稱「大賽」）不可能排除有受傷的危險。 2）參加「大賽」的條件之一是本人將放棄向主辦團體以及他們的雇員﹑大賽機構﹑主管人員﹑經理以及教練（以下簡稱「受保機構」）對參賽中本人可能遭到的意外傷害或損失提出索賠。本人同意在參賽期間，如有損壞比賽場地，本人將承擔所有責任。 3）根據此「豁免法律責任條款」（以下簡稱「條款」），本人將永遠放棄﹑解除和豁免所有因為參加本次「大賽」而引起的法律糾紛和訴訟，無論是已經發生或將要發生的﹑已知或未知的。 4）簽署此條款，本人將承擔所有參賽的風險。 本人將遵照主辦團體設立的比賽要求和規則。本人也不會在未經主辦團體同意的情況下撤出比賽。本人同意授權主辦團體在其商業廣告中使用本人的個人簡歷，所有參賽中的照片和錄像。本人保證所填寫的個人簡歷是屬實的。大賽過程中，本人有責任對簡歷中模糊和不正確的內容作出解釋和糾正。 本人在條款上的簽名，說明本人除了認同條款內容之外不承認其他任何人的口頭或書面說明。本人同意此條款受美國紐約州法律範圍的制約和解釋。在此，本人聲明是參賽選手並閱讀完條款的全文，理解並同意按條款要求執行。
